Survanta, with the generic name beractant, is a sterile, non-pyrogenic pulmonary surfactant derived from bovine lung extract. It is specifically formulated for intratracheal administration in neonatal intensive care settings. The suspension contains a mixture of phospholipids, neutral lipids, fatty acids, and surfactant-associated proteins, which work together to reduce surface tension within the alveoli, thereby preventing their collapse during respiration.

Indications:

  • Prevention (Prophylaxis): Administered to premature infants at risk of developing Respiratory Distress Syndrome (RDS), particularly those with evidence of pulmonary immaturity. In such cases, Survanta should be given as soon as possible after birth, preferably within 15 minutes.
  • Treatment (Rescue): Used for infants who have developed RDS confirmed by clinical and radiographic findings. For optimal efficacy, the first dose should be administered as soon as possible, ideally by 8 hours of age.

Dosage and Administration:

  • Dosage: The recommended dose is 100 mg of phospholipids per kilogram of birth weight, which equates to 4 mL/kg. Up to four doses can be administered within the first 48 hours of life, with each dose given no more frequently than every 6 hours. The need for additional doses is determined based on ongoing respiratory distress and radiographic confirmation of RDS.
  • Administration: Survanta is administered intratracheally using a 5 French end-hole catheter. Before administration, ensure proper placement and patency of the endotracheal tube. The suspension should be warmed to room temperature and gently swirled (not shaken) to ensure uniformity. Each dose is typically divided into four aliquots, with the infant’s position adjusted between each administration to facilitate even distribution within the lungs.

Safety and Precautions:

  • Survanta should only be administered in a highly supervised clinical setting by clinicians experienced in neonatal intubation and ventilator management.
  • Potential complications during administration may include bradycardia, oxygen desaturation, endotracheal tube blockage, or reflux of the suspension. Immediate measures, such as stopping the administration and providing appropriate medical intervention, should be taken if these occur.
  • There are no known contraindications to the use of Survanta.
