  • Mechanism of Action:
    • Lactulose is a disaccharide that is not absorbed in the small intestine. It passes into the colon, where it is broken down by bacteria into organic acids, which draw water into the colon, softening stools and stimulating bowel movements.
    • In hepatic encephalopathy, lactulose helps lower blood ammonia levels by promoting its excretion.

Dosage:

  • For Constipation: The typical adult dose ranges from 15–30 mL once or twice daily, depending on the severity of the constipation. Adjustments can be made based on the individual’s response.
  • For Hepatic Encephalopathy: The initial dose may be 30-45 mL three times a day, with adjustments made based on the patient’s ammonia levels and symptoms.

Serious side effects are rare but may include:

  • Electrolyte imbalances due to excessive diarrhea
  • Dehydration
